Lemay Avenue Health and Rehabilitation, LLC emerged as the largest nursing home in Larimer County for the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported.

The facility provided a total of 130 beds, outpacing the Larimer County average by 34 beds as of Q1 2026. The center served an average of 122.9 residents during this period.

Lemay Avenue Health and Rehabilitation, LLC achieved a perfect 5 out of 5 rating from CMS, surpassing Colorado’s statewide nursing home average of 3.2.

Creekside Village Rehabilitation and Nursing, LLC and North Shore Health & Rehab Facility ranked as the second largest facilities in the county for the quarter, each offering 120 beds.

During Q1 2026, Lemay Avenue Health and Rehabilitation, LLC incurred a total of $7,443 from one fine and faced a single penalty.

Larimer County had a total of 11 for-profit nursing homes as of the first quarter of 2026.

U.S. Census Bureau estimates show the country’s older adult population is growing, while the under-18 group declines. From 2020 to 2024, the number of Americans age 65 and older rose 13%, whereas those under 18 dropped by 1.7%. The older adult demographic has expanded substantially, rising from 4.9 million people in 1920 to over 61 million by 2024.

A 2021 analysis estimated nearly half of these older adults—around 28 million—could require long-term care services. The study also indicated that more than a third may ultimately need nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director for the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
