The facility operates as a for-profit nursing home.

During this period, Centre Ave Health and Rehabilitation, LLC achieved an overall CMS rating of five, surpassing Colorado’s average nursing home rating of 3.2. CMS evaluates homes using criteria including outcomes of health inspections, staffing ratios, and the quality of resident care.

CMS data notes the facility had an average of 86.1 residents daily while operating 90 available beds during the first quarter of 2026. The nursing home currently does not have a hospital-based affiliation.

U.S. Census Bureau estimates report ongoing growth in the nation’s older adult population, while younger age groups decline. From 2020 to 2024, the number of Americans age 65 and older grew by 13% and the number under age 18 dropped by 1.7%. Over the past century, the older adult population in the United States has risen from 4.9 million in 1920 to over 61 million by 2024.

According to a 2021 study, almost half of those over 65—approximately 28 million people—may need long-term services and supports, with over a third potentially requiring nursing home care at some point.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
